Access to Forms

The Superior Court of Orange County offers several options to assist the public in completing court forms:

  • State Judicial Council Approved Forms(link is external): All statewide approved forms are available for Adoptions, Civil, Conservatorships, Criminal, Guardianships, Family Law, Juvenile, Name Change, Probate, Small Claims, and Traffic.
  • Locally Approved Forms: In addition to forms approved by the State Judicial Council, the Superior Court of Orange County has approved a variety of forms, some of which are fillable, giving you a larger selection of forms to assist you.
  • Locally Approved Form Packets: In addition to the Court's Self-Help Centers form packets, which contain the required State Judicial Council Approved Forms and Locally Approved forms for various case types, the various case types have case-type specific form packets created to assist the public in completing these forms.
  • Self-Help Form Packets: The Court's Self-Help Centers provide a variety of form packets (which contain the required State Judicial Council Approved Forms and Locally Approved forms for various case types). Also, our Self-Help Centers can provide form packets, document review and workshops to self-represented litigants.
