NC-100-INFO Instructions for Filing a Petition for Change of Name Explains what will happen in your name change case and what forms to use. Also gives information about changing a child's name and other important information.
L-3008 Name Change Notice of Related Cases Tell the court about another case that may be related to a name change request.
CM-010 Civil Case Cover Sheet Tells the court some basic information about a case being filed. Filed at the beginning of all civil cases except family law cases.
NC-100 Petition for Change of Name Start a case to change your name, your minor child's name, or both.
NC-110 Name and Information About the Person Whose Name Is to Be Changed Tells the court about the person who is asking for a name change. Use one form for each person whose name you want to change. This is always attached to a NC-100, NC-200, or NC-500 form before it is filed.
NC-120 Order to Show Cause—Change of Name Tells anyone viewing the form that you are asking for a name change and lists where the form will be published. This is also used by the court to set the date, time, and location of the hearing.
NC-130 Decree Changing Name States the court’s decision (order) about your name change.
L-1116 Newspapers of General Circulation (Adjudicated) Lists newspapers approved by the court to publish legal notices in Orange County.

CM-010 Civil Case Cover Sheet Tells the court some basic information about a case being filed. Filed at the beginning of all civil cases except family law cases.
NC-400 Confidential Cover Sheet—Name Change Proceeding Under Address Confidentiality Program (Safe at Home) Use this as a cover sheet when you file papers to start a name change case if you are in the Safe At Home program and are asking the court to keep your case confidential.
NC-100 Petition for Change of Name Start a case to change your name, your minor child's name, or both.
NC-110 Name and Information About the Person Whose Name Is to Be Changed Tells the court about the person who is asking for a name change. Use one form for each person whose name you want to change. This is always attached to a NC-100, NC-200, or NC-500 form before it is filed.
NC-120 Order to Show Cause—Change of Name Tells anyone viewing the form that you are asking for a name change and lists where the form will be published. This is also used by the court to set the date, time, and location of the hearing.

NC-420 Declaration in Support of Application to File Documents Under Seal in Name Change Proceeding Under Address Confidentiality Program (Safe at Home) Explain the facts that support your request to keep the papers in your name change case confidential.
MC-025 Attachment Gives you more space to complete any Judicial Council form. This is always attached to another form or court paper before it can be filed in court.
NC-425 Order on Application to File Documents Under Seal in Name Change Proceeding Under Address Confidentiality Program (Safe at Home) Tells you whether a judge granted the request to keep the papers in your name change case confidential.

NC-500 Petition for Recognition of Minor's Change of Gender and Sex Identifier and for Issuance of New Birth Certificate and Change of Name Start a case to get a court decision (order) recognizing your child's gender, and (if you want) to change their name.
NC-520 Order to Show Cause—Recognition of Minor's Change of Gender and Issuance of New Birth Certificate Tells anyone viewing the form that you are asking for a name change for a child. Also tells the child’s parents or grandparents that you are asking the court to recognize the child’s gender.
NC-530 Order Recognizing Minor's Change of Gender and Sex Identifier and for Issuance of New Birth Certificate States the court’s decision (order) about the child’s gender and any name change requested, and orders that a new birth certificate be issued.
NC-121 Proof of Service of Order to Show Cause (Change of Name) Tells the court  that you had the Order to Show Cause and other name change forms delivered to (served on) the other parent or relatives in a case. Lists the papers that were served and tells who they were served on, where, when, and how they were served, and who served them.
