Create Court Forms

SmartForms: SmartForms Technology adds prompts to assist you in completing fillable forms at no cost. SmartForms also allow for data validations, calculations, auto-filling and online submission to the court. Ideal for the user who is familiar with the forms and procedures required for their case. The Superior Court of Orange County is in the process of converting several local and state forms to make use of SmartForm Technology. As these forms become available, they will be added to the court’s website. If you do not find the form you need, refer to the state and local forms, or the tutorial form program.

State Judicial Council Approved Fillable Forms: Fillable forms are available for free. There are no prompts to assist you with the completion of the forms. All state approved forms are available for Adoptions, Civil, Conservatorships, Criminal, Guardianships, Family Law, Juvenile, Name Change, Probate, Small Claims, and Traffic.

Locally Approved Fillable Forms: In addition to forms approved by the state, the Superior Court of Orange County has also approved a variety of free fillable forms, giving you a larger selection of forms to assist you. Some commonly used state and local forms have also been grouped together in "packets" with instruction sheets, taking the guess work out of which forms you need to use.

Want to eFile your document? The Superior Court of Orange County currently allows eFiling for the following documents (more documents will be added in the future). Some forms and documents will require the payment of a convenience fee if you want to submit them to the court electronically.

• eFiling is available for all Limited and Unlimited Civil documents.
• eFiling is required for all Complex Civil documents.

• eFiling is available for forms completed using SmartForms.

• eFiling is available for Probate documents.

• The I-CAN! Legal tutorial program gives you the option to eFile a Small Claims claim or Countersuit (Defendant’s Claim), or
• Complete your Small Claims claim or Countersuit (Defendant’s Claim) using SmartForms Technology and eFile your document to the court.
