Jury staff of the superior courts do not ask past or prospective jurors for information regarding credit card, bank account information and Social Security numbers. Please do not provide this type of information to anyone claiming to be associated with Jury Services. Please contact our office if you receive this type of request.

Juror Affidavit Questionnaire
The Juror Affidavit Questionnaire is sent to prospective jurors with their
summons. The online version of the form may be used by jurors who did not return the questionnaire
as requested.
If you are reporting for jury service but have not returned the Juror Affidavit Questionnaire, please print out this form, complete it and give it to the jury clerk on your reporting date.
