The Juror Affidavit Questionnaire was sent to prospective jurors with their summons. This form may be used by jurors
who did not return the questionnaire as requested.
- If you are reporting for jury service but have not returned the Juror Affidavit Questionnaire,
please print out this form, complete it and give it to the jury clerk on your reporting date.
